8 years old male presented with fever, pallor, bleeding tendency for 2 weeks duration, on examination splenomegaly was found.
The patient CBC shows anaemia with thrombocytopenia, but normal total WBC count for age.
His blood film shows about 60% leukaemic cells some with convoluted nucleus .
Bone marrow aspiration show hypercellular smears with massive infiltration by blasts and hypergranular promyelocytes, some of these leukemic cells contain psuedo-Chediak Higashi granules (some are giant others are small, single or multiple pink or purple cytoplasmic granules), but lack auer-rods.
